Advanced Nurse Practitioner – Chertsey 
Flame Health are currently recruiting for an Advanced Nurse Practitioner to join our client who is committed to serving the community. They aim to make their services exemplary in both clinical and operational aspects. They will show leadership in identifying healthcare needs to which we can respond and in determining the most cost-effective way of doing so.
We are looking to recruit a competent candidate that supports in maintaining and extending their skills in accordance with the needs of the people our client serve. You will recognise the commitment from the staff to meeting the needs of the patients.
Main responsibilities:

• You will develop, implement and evaluate a seamless specialist service, ensuring that patients receive the highest standard of clinical care
• You will lead the identification and clinical assessment of patients who will benefit from advanced discharge or admission avoidance with care provided, in their own home by the Rapid Response multi-disciplinary team
• You will demonstrate a high level of expertise in this field providing advice, education and support to health care professionals and patients
• You will actively case find and promote the Rapid Response service work closely with Consultants, GPs and other referrers and undertake clinical audits
• You will be responsible for contributing to the on-going development of clinical practice, research and standards of care within the service, as well as policies, protocols, guidelines and service developments
• You will ensure the safety and well-being of service users, and always work in a manner that promotes dignity and human rights through the adoption of person centred care principles

It is essential that for this position you are a qualified Advanced Nurse Practitioner registered with the NMC.
