GP Prescribing Errors
The National Prescribing Centre (NPC) has said in prescribing guidance published last week that GPs should enlist the aid of pharmacists to help minimise prescribing errors,
The NPC guide follows studies that revealed GPs have a prescription error rate of 7.5 per cent, with errors in four drug classes – antithrombotics, anticoagulants, NSAIDs and diuretics – responsible for half of preventable medication-related hospital admissions.
Common reasons for prescribing errors included lack of knowledge about the patient or the medicine, checking lapses caused by heavy workload, and communication problems, the NPC added.
Tips to prevent errors cited in the guidance included actively checking prescriptions for errors before handing them to patients and establishing safety SOPs when repeat prescribing.
