Practice Nurse - Primary Care Extended access - Hampshire Area

Part time/Full time/Locum considered
Salary £26,000 -£58,500 depending on hours worked and experience (Ranging from £20 - £25 per hour)

Flame Health are currently working with a growing healthcare company, dedicated to offering quality solutions and services to NHS and private healthcare partners within the Hampshire area

The Practice Nurse will be responsible for the provision of safe and effective primary care appointments with full access to patient medical records.
You will provide appointments in an evening and at weekends to local patients referred by their own GP surgery and 111.
You will take a pro-active approach to your own CPD, contribute to continually improving the clinical effectiveness of the clinics and work collaboratively and supportive with all colleagues.

You will have a choice of shifts across the week and from a number of locations: In the Hampshire area

· Monday-Friday, with shift lengths varying from between 6.30pm to 11pm (Flexible hours available)
· Saturdays and Sundays and Bank Holidays 8am to 11pm.(Flexible hours available)
This is a great opportunity to work in a portfolio role, with a local practice, A&E or Urgent Care service.

Key Accountability:

Clinical Duties

Level 1 - Cervical Screening & dressings

Level 2 - Level 1 plus family planning to include contraceptive initiation and review, Immunisations

Level 3 -Chronic Disease Management (Asthma, COPD, Diabetes, Hypertension, CVD)

-You will provide Treatment Room services and others, dependant upon Competency level. To include: wound care, cervical cytology, swabs, family planning such as contraceptive initiation and checks, Chronic Disease Management (Asthma, COPD, Diabetes)
· Screening. Undertake opportunistic screening including BP, height, weight, BMI, urinalysis, smoking status and alcohol intake.
· Health Promotion.
· Infection Control. Ensure that the highest infection control standards are met.
· Emergency Management. Be competent in anaphylaxis and resuscitation techniques
· Safeguarding. Be aware of recognising the need for safeguarding children and adults, raising concerns appropriately.
· Administration and paperwork.
· Personal CPD (continuing professional development), a mix of in-house events, private study and external educational events are expected to remain up to date as required by your professional registration.
· To be professionally and legally responsible and accountable for all aspects of clinical decision making within your clinical competence

· To provide mentor ship and support to junior staff when required
· To participate in regular performance review with your designated Clinical Lead

It is expected that the post holder will:

· Communicate with the Governance Lead and support the delivery of a high quality service
· Work with the Management team to ensure training, complaints, audits and other professional service development activities are achieved

Skills & Experience - Essential

· Registered with NMC / HCPC
· Experience working in a primary care
· At least three years post qualified
· Competent at providing treatment room tasks such as immunisations, wound care, chronic disease management
· You will be expected to ensure that mandatory education is kept updated · Excellent communication and interpersonal skills
· Clear understanding of professional responsibility and accountability.
· Must be authorised to work in the EEC


· Minor illness qualification or similar
· Experience of Chronic Disease Management
· Able to use EMIS clinical IT system
